You may submit this form multiple times if you have more than 25 staff members. Alternatively, you may also attach a staff list containing the contact fields below in Microsoft Excel format by clicking the “Browse” button on the bottom of this form. Please add ALL staff members including employees and contractors on company payroll as well as 3rd parties that serve in the capacity of an employee. Each staff member listed must have a unique email address (no duplicate addresses).
Email Account Information
We will need access to all current 3rd party web-based email accounts (gmail, yahoo, etc) used internally by staff in order to migrate them to your new email system. This form does NOT need to be completed if all staff & company email addresses already reflect your website domain (e.g. John@FamilyMedical.com).
